Basic National Social Security Scheme Regulations, 2011 (L.I. 1989)
Regulation 32 An application for payment of survivors' benefit, shall be accompanied with (a) a statutory declaration or affidavit and a passport size photograph of the applicant duly endorsed by the official before whom the statutory declaration or affidavit was made; or (b) a valid national identification with a photograph in the form of a passport, driving licence, national identification card or a national health insurance card.
